INCIDENT REPORT FORM

The following persons would be required to fill out different sections of the form as follows:

(1) Person who was injured or involved in accident, near miss or incident.

(2) Each witness to the incident needs to fill out the witness section of the form.

(3) The supervisor/foreman (whomever from Contractor's side would be investigating the incident) needs to fill out one section

(4) The Construction Manager fills out one section.

In the event of any significant incident, accident or near miss that occurs during Contractor's work on the job, the following report form must be filled out and submitted to the Construction Manager by Contractor.    THIS REPORT MUST BE SUBMITTED WITHIN 24 HOURS OF INCIDENT.
